The round was led by Aldrich Capital Partners.
The company intends to use the funds to hire additional care coordination staff, sales and marketing personnel, and developers to expand its technology capabilities.

The company enables providers to deliver continuous, comprehensive care by establishing care management programs for patients with chronic health conditions. The solution combines a SaaS platform with care coordination services for chronic care management, remote patient monitoring, and behavioral health integration.
